NEW PLATES
• The original title
• Current NY State Insurance ID card
• The Bill of Sale
• Your NY State driver license, permit, non-driver ID or 6 points of id and attach proof list.
• A completed Vehicle Registration Application
• If you register for a business or organization, also bring proof of incorporation.
• Proof of sales tax payment / sales tax form (DTF 802 or DTF 803 or DTF 804)
TRANSFER PLATES
• Need a copy of registration of the plate number you’re transferring from to new vehicle.
• Current NY State Insurance ID card ( MUST SAY replacement vehicle)
REPLACEMENT PLATES
• A completed Vehicle Registration Application MV-82
• Remaining Plate
• MV-78B form (you must obtain this form from your local prescient if your plates were stolen or if both plates were lost)
• Insurance card (The effective date on the insurance card must be on or 45 days prior to the date of registration) (*all three)
• Your NY State driver license, permit, non-driver ID
Plate Surrenders
• Both plates
• MV-78B form (you must obtain this form from your local precinct if one or both plates were lost or stolen)
